Special emphasis will be on the dangers of lead, mold, and pesticides and how to decrease the exposure to allergens and irritants associated with asthma. This is a collaborative project with city and county health departments, EPA, IDEM, Step Ahead Environmental Justice –IUN, Head Start and many other social and health agencies. Community Health Worker Program HealthVisions Midwest Lead: Sr. Annemarie Kampwerth Goal: To increase the capacity of Lake County, Indiana to reduce health disparities, by developing and implementing a Community Health Worker education program and setting up practicum experiences for students and providing job opportunities for graduates. Maternal Child Health Network of Lake County HealthVisions Midwest Lead: Clementine DuBose Goal: A coordinated effort has been spearheaded to bring all stakeholders, who interact with pregnant clients into a network. Four care coordination teams are in the forefront of this effort, Great Beginnings, NATALE, Gary HHS/MCH (formerly known as Project Pre-cint), and Healthy Start. Each team is working within their target area to recruit and provide care coordination of services. Everyone is working to ensure services to clients are not duplicated within the Network. Partnerships are being formulated with other area entities as well. This project is part of HealthVision's Kids Health Initiative aimed at improving the health status of minority and low-income youth in communities we are currently serving. The program goal is to: 1. Increase physical activity among youth. 2. Increase consumption of fresh fruits and vegetables. 3. Volunteers from interfaith churches and parishes are trained to improve the quality of life for their fellow members and community members who are suffering from chronic diseases. Health Disparity Coalition of Color HealthVisions of Fort Wayne Lead: Renetta Williams Goal: A multicultural action plan to achieve health parity in communities of color. Develop an Allen County Health Model to provide quality health care for all. Lifetime Fitness Program HealthVisions of Fort Wayne Lead: Renetta H. Williams Goal: The Lifetime of Fitness, a program of HealthVisions of Fort Wayne partnered with area neighborhood organizations to provide access to healthier lifestyle choices. This program helps organizations set up low or no cost exercise sites and/or nutrition and health education to adults, children, and youth in the Central and Southeast communities of Fort Wayne to combat the ever increasing epidemic of obesity and concurrent chronic diseases. We have over 100 people attending weekly at all sites. A $20,000 grant was submitted to the City of Fort Wayne to combine the fitness, gardening, and the Down Home Healthy Living (DHHL) program to address strategies for disease prevention and early detection. Management FWAACA: This is a group under the management of HealthVisions Fort Wayne. The FWAACA has organized a Prevention Clinic in collaboration with the Allen County Health Disparity Coalition, Lafayette Medical Center, and the Area Health Education Center to provide free and low cost screening, education, case management and follow-up for cancer, diabetes, high blood pressure checks, cholesterol, HIV/AIDS, depression, visions, lead poisoning and sickle cell. The initial start up date was Wednesday, January 18, 2006. The Clinic will be open every 3rd Wednesday 5:00 P.M to 7:00 P.M. at 2700 Lafayette Street –Suite 100. Starke County, IN [ top of page] Living for Life HealthVisions Midwest Lead: Kathy Robb Goal: To reduce health disparities among the rural poor population of Starke County, Indiana by providing a program focusing on "living a healthy lifestyle. The program will be approximately 10 weeks long and will include weekly educational sessions. Each session will include a 30-minute lecture topic (guest speakers from various community agencies will be doing the lectures), the nutrition tip of the week, a weekly healthy recipe and an exercise tip of the week. We will also offer weekly "weigh in" and track the amount of weight lost be different organizations and/or groups as well as the community. Starke County Memorial Hospital, United Way, the local radio station and other local agencies are partners in this project. LaPorte County, IN [ top of page] Healthy Communities of LaPorte County Goal: As a partner of Healthy Communities of LaPorte County we collaborate with other organizations in LaPorte County in finding solutions to issues that impact the emotional, physical and spiritual health and overall quality of life of the citizens of that county.We are specifically involved in the Health Council and establishing and implementing a child maternal health network for the county with specific programs for pregnant women. St. Joseph, Elkhart, Marshal Counties, IN [ top of page] Medical Mobile Unit Goal: The Mobile Medical Unit is Saint Joseph Regional Medical Center’s 40-foot-long clinic on wheels. The Mobile Medical Unit (MMU) brings unique healthcare to the communities served by SJRMC, expanding access in both urban and rural areas, while helping to meet the needs of at-risk individuals who lack transportation or adequate access to medical offices or clinics. HealthVisions Midwest is collaborating with St. Joseph by helping to fund this unit at sites in Plymouth, especially for the Spanish speaking population and some underserved areas of Mishawka. The state-of-the-art Mobile Medical Unit houses a medical an exam area, a mammography unit, plus an area for intake, information, and health education. The MMU will also be used in partnership with area cancer programs, women’s health services, community programs, and corporate wellness programs. East St. Louis, IL [ top of page] Faith in Action to Serve and Transform (FAST) HealthVisions East St. Louis Lead: Paula Wills Goal: Inspired by the compassion of Christ, FAST provides interfaith, volunteer based non-medical support and care giving services to individuals who are homebound, frail, elderly, disabled or afflicted with chronic illnesses in the Greater East St. Louis community.
