What's a Community Health Worker?

  • Community Health Workers serve as a link between a community and its health and social service systems.
  • They improve access to and delivery of health services.
  • Build capacity for individuals/families to promote their own health and well-being.

Who should attend?

  • Individuals who wish to pursue a career as a certified community health worker and enter the healthcare and social service arena.
  • Healthcare facilities who wish to certify staff as Community Health Workers to work in a variety of settings to increase access to health services and decrease hospital readmissions.
  • Individuals must be 18 years or older and must have completed High School or equivalent to qualify.

Community Health Worker Training

HealthVisions Midwest (HVM) completed its first Certified Health Worker Training on April 7, 2017 and a Leadership Training on April 27, 2017. HealthVisions became the first vendor to have a Certified Curriculum for Community Health Workers (CCHW). HVM lost no time in putting its first class together.

Through a partnership with Esperanza Ministries, the Morales group and the Indiana Department of Workforce Development, HVM received Skill Up grant monies to conduct the training. During the week of April 3-7, 2017, 12 Latino women participated in a 40-hour curriculum, were tested and received their certificates. The Skill Up grant also calls for a 30-hour Leadership Training. The CCHWs completed the Leadership portion on April 21, 2017.

Partnering with Work One of Lake County, IN, the trained individuals will enter into a 20-hour “externship” in a healthcare or community health setting. HealthVisions Midwest and Work One are partnering with Franciscan Health to give several of the CCHWs a more extensive externship that may culminate in a job.
